For your convenience, you can complete your intake paperwork online through a secure client portal. Upon setting up your first appointment, you will be provided a link to the portal. 

If you wish to complete your initial intake paperwork in person, please arrive 15 minutes before your first appointment to complete necessary paperwork.

Insurance & Payment

I accept private payment for services, or I can file a claim with your insurance provider.

Please contact your insurance company to learn about your coverage for mental health services, including information about applicable deductible and co-payment responsibilities. You can verify your benefits by calling the customer service phone number located on the back of your insurance card. 

Dr. Harlow is an in-network provider with Blue Cross Blue Shield, Aetna, Midland's Choice, and Medicare.

Please bring your insurance card and photo ID to your first appointment. I will scan a copy if you plan to have your visits filed with your insurance provider.

I accept cash, personal check, or credit cards. I also accept payment through Health Savings Accounts and Flexible Spending Accounts.

Good Faith Estimate (GFE)

The No Surprises Act went into effect January 1, 2022 which requires me to provide a Good Faith Estimate of the costs of care.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

  • I will give you a Good Faith Estimate in writing at least 3 business days before your scheduled session, or within 1 day if you the schedule appointment is only a few days out. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
